RESUMO

The IAEA is currently coordinating a multi-year project to update the TRS-398 Code of Practice for the dosimetry of external beam radiotherapy based on standards of absorbed dose to water. One major aspect of the project is the determination of new beam quality correction factors, k Q , for megavoltage photon beams consistent with developments in radiotherapy dosimetry and technology since the publication of TRS-398 in 2000. Specifically, all values must be based on, or consistent with, the key data of ICRU Report 90. Data sets obtained from Monte Carlo (MC) calculations by advanced users and measurements at primary standards laboratories have been compiled for 23 cylindrical ionization chamber types, consisting of 725 MC-calculated and 179 experimental data points. These have been used to derive consensus k Q values as a function of the beam quality index TPR20,10 with a combined standard uncertainty of 0.6%. Mean values of MC-derived chamber-specific [Formula: see text] factors for cylindrical and plane-parallel chamber types in 60Co beams have also been obtained with an estimated uncertainty of 0.4%.

The validity of four indicators to predict successful duodenal feeding tube placement was evaluated in a prospective trial. Data were collected on each indicator at prepyloric (< or = 65 cm) and postpyloric (> or = 75 cm) feeding tube lengths. Feeding tubes were placed in 106 patients. Eighteen feeding tubes were located in the stomach, and 88 were in the duodenum. Auscultation (progression of loudest sound locations from the left to the right abdomen) had a positive predictive value of 85% (negative predictive value, 31%). The vacuum effect (a change from 40 mL of aspirated air to < or = 10 mL after 60 mL of air instillation) had a positive predictive value of 86% (negative predictive value, 45%) and was significantly correlated with duodenal placement (p = .02). Aspirate was present at prepyloric and postpyloric lengths in 35 cases. Ten of these 35 cases had the defined change in pH from < or = 4.0 to > or = 6.0 (positive predictive value, 100%; negative predictive value, 28%). The positive predictive value of color (a change to yellow) was also 100% (n = 11); the negative predictive value was 29%. The low negative predictive values of the indicators suggest that the absence of defined changes is of no assistance in discriminating between stomach and duodenal placement. A positive auscultation or vacuum effect test is not conclusive for duodenal placement. A positive pH or color change test may obviate the need for a confirmatory radiograph.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMO

Quality of work life (QWL) programs can improve employee morale and organizational effectiveness. But implementing a successful QWL effort in a healthcare setting is not easy because of the unique administrative structure and inherent complexities of modern hospitals. This article describes a joint union-management quality of work life program that was carried out in a large urban medical center. Despite employee enthusiasm for the program and a major commitment of financial and human resources over its three-and-a-half-year course, the program failed to achieve long-term changes in the hospital. The shortcomings of the QWL program are analyzed and recommendations for future practice are described.
